    Job Summary

    This position is responsible for coordination of team members, monitoring documentation, and coordination and training of the home health software and OASIS.  Provides direct supervision of Registered Nurses, Licensed Practical Nurses, or Therapists as assigned and indirect supervision of Home Health Aides.  This position is responsible for delivery of quality service, promoting good public relations, and for upholding the mission and standards of the facility.



    1. Monitors, coordinates, and evaluates client care given by assigned clinicians.
    2. Demonstrates leadership skills.
    3. Hires and supervises licensed staff in Lincoln and in branch offices as assigned.
    4. Ensures assigned visiting staff follows federal/state/other third party payers’ regulations regarding client care as well as policies and procedures of Tabitha Health Care as designated.
    5. Coordinates and provides computer training to home health care clinicians as assigned.
    6. Identifies and addresses concerns related to computer documentation
    7. Provides computer software support for Home Health Office staff and licensed clinicians as assigned.   
    8. Approves Home Health purchases, invoices, employee time sheets, and mileage in assigned regions.
    9. Provides supervisory on call on a rotating basis.
    10. Coordinates activities of team members (Visiting Nurses, Home Health Aides, Therapists) including necessary scheduling of team members including weekend visits and on call rotation.
    11. Performs visiting nurse or therapy duties as assigned.
    12. Adheres to the clients Home Health Care Bill of Rights.
    13. Follows all safety and infection control policies and practices established by Home Health Care and regulatory agencies.
    14. Knowledgeable and adheres to disaster procedures during drills and actual emergencies, e.g., fire, tornado, bomb threat, and evacuation.
    15. Assists with new staff orientation and with coordination of students as assigned.
    16. Informs the Assistant Administrator of any problems or circumstances which may compromise the integrity of Tabitha Health Care Services.
    17. Demonstrates and promotes communication skills.
    18. Participates in organizational and departmental goals and performance improvement activities.
    19. Participates in personal and professional growth by attending in-services/educational workshops and scheduled department meetings.
    20. Adapts to a changing work load and work environment.
    21. Maintains a positive outlook toward employer and work responsibilities.
    22. Assists in maintaining Tabitha's positive image in the community.
    23. Organizes and prioritizes work to complete responsibilities in established timeliness.
    24. Assures delivery of optimum service by following assigned schedules.
    25. Dresses and conducts self in an appropriate manner according to HHC Agency policy.
    26. Performs other responsibilities and special projects as assigned by the supervisor.


    Provides information related to Home Health care upon supervisor's request.



    Utmost accuracy is essential requiring concentration and close attention to detail when performing team leader functions.  Teamwork is involved in maintaining records.  Visiting Nurse functions requires physical assessment skills.  Knowledge of and ability to apply community health nursing principles and practices is required.  Knowledge of teaching/learning principles, environmental hazards and good safety practices.  Knowledge of Medicare Home Health Regulations including the Outcome and Assessment Information Set (OASIS) requirements needed.

    Normal office environment.  The emphasis on accuracy can cause fatigue and mental strain for team leader functions.


    Visiting Nurse or Therapist duties require:

    Working in clients' places of residence including apartments, town houses, duplexes, and houses.  May care for clients with infectious disease so must observe Standard Precautions/Infection Control procedures.  Travel is required from residence to residence in all kinds of weather.  Must have a reliable means of getting to each client's residence on a timely basis.



    Nursing Clinical Coordinator:  RN graduate from accredited school of nursing required.  BSN preferred. Education in community health preferred. Three years of RN nursing experience of which one year is RN Supervisory experience required. Home health nursing experience preferred.


    Therapy Clinical Coordinator:  Graduate from accredited school of therapy required.  Education in community health preferred. Three years of experience of which one year is supervisory experience required. Home health experience preferred.



    Sedentary position involving reading clinical records.  When performing functions of a visiting nurse, must have ability to lift up to 50 pounds, twist upper body and bend at waist while transferring, moving, turning, and supporting clients.


    Equipment Operation

    Use of telephone, cell phone, computer, and copier.  When performing Visiting Nurse duties, use of the following equipment is required:  home blood glucose monitoring equipment, standing and chair scales, home ventilator, infusion pump, vascular pump, pocket Doppler, mechanical lift (e.g.  Hoyer), wheel chair, walker, cane splints, brace, commode, thermometer, sphygmomanometer, and stethoscope.
