    Job Summary

    This position is responsible for assessing, implementing, coordinating, evaluating, and documenting comprehensive in-home services for Home Health Care clients. This position is responsible for delivery of quality service, promoting good public relations and for upholding the mission and standards of the facility.     1. Demonstrates commitment to Tabitha’s Crew Code of conduct.
    2. Receives and processes referrals for Home Health Care services as assigned.
    3. Makes evaluation home visits to identify needs and appropriateness for home health care.
    4. Admits clients for services and completes appropriate admission documentation according to agency policy and procedures and state/federal regulations.
    5. Maintains clinical record documentation according to agency policy and procedures and state/federal regulations.
    6. Provides skilled observation and/or care to clients requiring intermittent professional nursing services.
    7. Teaches and supervises Home Health Aides, family members, and non-paid caregivers providing client care according to agency procedures and state/federal regulations.
    8. Teaches and supervises Home Health Aides in assisting with medications per agency policies and procedures, and state/federal regulations, and monitors the client for medication therapeutic response and side effects.
    9. Coordinates the client's care and ensures that appropriate treatment is obtained.
    10. Adheres to the client’s Home Health Care Bill of Rights.
    11. Provides backup staffing as needed for regular on-call and weekend nursing staff as assigned.
    12. Follows all safety and infection control policies and practices established by Home Health Care and regulatory agencies.
    13. Organizes and prioritizes work to complete responsibilities in established timeliness.
    14. Demonstrates and promotes communication skills.
    15. Adheres to and has knowledge of HHC policies and procedures.
    16. Assures delivery of optimum service by following assigned schedules.
    17. Participates in organizational and departmental goals and performance improvement activitiesKnowledgeable and adheres to disaster procedures during drills and actual emergencies, e.g., fire, tornado, bomb threat, and evacuation.
    18. Participates in personal and professional growth by attending in-services/educational workshops and scheduled department meetings.
    19. Assists in maintaining Tabitha's positive image in the community.
    20. Dresses and conducts self in an appropriate manner according to HHC Agency policy.
    21. Adapts to a changing work load and work environment.
    22. Maintains a positive outlook toward employer and work responsibilities.
    23. Performs other responsibilities and special projects as assigned by the supervisor.



    • Provides health teaching, blood pressure/blood sugar screening (or immunizations) and appropriate referrals at agency health clinics as assigned.
    • Assists in orienting new nursing and Home Health Aide staff as directed by supervisor.
    • Provides information related to home care upon supervisor's request.




    Nebraska Registered Nurse or Licensed Practicing Nurse license required.

    Valid Nebraska Driver's license and the state required minimum automobile insurance coverage required.

    Current Healthcare Provider or BLS CPR/AED certification required.



    Excellent physical assessment skills required.  Knowledge of and ability to apply community health nursing principles and practices.  Knowledge of teaching/learning principles, environmental hazards and good safety practices.

    Works in clients' places of residence including apartments, town houses, duplexes, and houses.  May care for clients with infectious disease so must observe Standard Precautions/Infection Control procedures.  Travel is required from residence to residence in all kinds of weather.  Must have a reliable means of getting to each client's residence on a timely basis.  May be exposed to pets and tobacco smoke in client’ residence.



    Graduation from NLN accredited school of nursing required.  BSN preferred.

    Two years of acute or skilled care nursing experience within the last three years required.

    Home Health experience preferred.



    Must have ability to lift up to 50 pounds, twist upper body and bend at waist while transferring, moving, turning, and supporting clients.


    Equipment Operation

    Use of computer for clinical record documentation as assigned.  Use of cell phone.  Use of home blood glucose monitoring equipment, standing and chair scales, home ventilator, infusion pump, vascular pump, pocket Doppler, mechanical lift (e.g., Hoyer), wheel chair, walker, cane, splints, brace, commode, thermometer, sphygmomanometer, stethoscope and oximeter. Telehealth Equipment.
