This position is responsible for providing personal care, emotional support, assistance with activities of daily living, and basic therapeutic care to clients. Maintains records of patient care. This position is responsible for delivery of quality service, promoting good public relations, and for upholding the mission and standards of the facility. Performs clients' personal care including activities of daily living and therapeutic care according to the client's written plan of care as instructed.
Identifies client/family teaching needs to the nurse to maintain the client in their own home.
Performs client care in client’s home, Tabitha nursing home, or other facility, as assigned and instructed.
Assists in monitoring the client’s use of equipment according to the written plan of care.
Provides emotional support to client and family.
Presents self in appropriate manner.
Adheres to the client’s Bill of Rights.
Maintains strict level of confidence concerning Tabitha’s clients.
Participates as team member to ensure that the department workloads and goals are met.
Observes and reports changes in the client's condition and problems to the client's nurse or supervisor according to procedures.
Adapts to changing work load and work environment.
Documents care provided, observations made, and communications with the visiting nurse on the client’s clinical record.
Organizes and prioritizes work to complete tasks neatly and accurately within established time lines.
Follows safety and infection control policies and practices of the agency.
Maintains a safe and healthy environment for the client, recognizing and reporting unsafe conditions.
Assures delivery of optimum service by reporting to work on time and as scheduled.
Participates in personal and professional growth by attending mandatory in-services and scheduled department meetings.
Performs other duties and special projects as assigned by supervisor.

Assists and/or gives medications and treatments per policy and procedure.
Assists with orientation of new employees when requested.
Current Nebraska Certified Nursing Assistant Certification.
Valid Nebraska Driver's license and the state required minimum automobile insurance coverage required.
Current Healthcare Provider or BLS CPR/AED Certification required for Home Health Aides working in Medicare Certified programs, otherwise highly recommended.
EDUCATION/TRAINING AND WORK EXPERIENCE
Must be at least 18 years of age. State approved Home Health Aide training or competency testing and Medication Aide training /competency testing prior to client care required. Prior completion of state approved Home Health Aide and Medication Aide training courses preferred. Experience as a Home Health Aide or Nursing Assistant preferred.
KNOWLEDGE, SKILLS AND ABILITIES
Must have ability to: speak, read and write English, maintain neat and accurate records, relate well with families in the community, work well with all levels of staff, perform CPR (if trained to do so)
Must have knowledge of: normal nutrition and fluid intake, good health habits, and principles of good hygiene, normal growth and development, environmental hazards and good safety practices, factors that create stress and how behavior may reflect this
Required to have a telephone for availability for schedule changes. Works in clients' places of residence including apartments, town houses, duplexes, houses, and nursing facilities. May care for clients' with infectious disease, so must observe Standard Precautions/Infection Control procedures. Travel is required from residence to residence in all kinds of weather. Must have a reliable means of getting to each client's residence on a timely basis. May be exposed to pets and tobacco smoke in client’s residence.
PHYSICAL REQUIREMENTS NEEDED TO PERFORM THIS JOB
Must have ability to regularly lift 50 pounds; twist upper body and bend at waist while transferring, moving, turning, and supporting clients. Must stand, bend at waist while giving baths to clients at low bed heights.
Use of mechanical lifts (e.g. Hoyer), braces, splints, wheel chair, walker, canes, commode, standing and chair scales, thermometer, sphygmomanometer, and stethoscope.
