This position is responsible for immediate evaluation and direct care of clients on a PRN basis, providing continuity of care on a 24-hour basis, evaluating referrals after hours and providing program information as needed.
This position is responsible for delivery of quality service, promoting good public relations, and for upholding the mission and standards of the facility.
Current Nebraska Registered Nurse licensure.
Valid Nebraska driver's license and state required minimum automobile insurance coverage required.
KNOWLEDGE, SKILLS AND ABILITIES
Excellent physical assessment skills required. Knowledge of and ability to apply hospice nursing principles and practices. Knowledge of teaching/learning principles, environmental hazards and good safety practices. Ability to:
- exercise independent nursing judgment
- supervise and assign work to others
- perform CPR
- assemble, record, and present data in a concise, organized systematic manner
- teach clients and families
Works in clients' places of residence including apartments, town houses, duplexes, and houses. May care for clients with infectious disease so must observe universal precautions/Infection Control procedures. Travel is required from residence to residence in all kinds of weather. Must have a reliable means of getting to each client's residence on a timely basis. May be exposed to pets and tobacco smoke in client’s residences.
EDUCATION/TRAINING, AND WORK EXPERIENCE
Graduation from NLN accredited school of nursing required.
Two years of acute care/long term care nursing experience within the last 5 years required. Emergency Room/Critical Care experience preferred.
Experience or education in community health preferred.
PHYSICAL REQUIREMENTS NEEDED TO PERFORM THIS JOB
Must have ability to lift up to 50 pounds, twist upper body and bend at waist while transferring, moving, turning, and supporting clients. Travel required within the community served by the assigned team. Must have ability to be mobile within the assigned service area.
Use of dictating machine or notebook computer to record nursing notes and pager. Use of home blood glucose monitoring equipment, standing scales, infusion pump, mechanical lift (e.g., Hoyer), wheel chair, walker, cane, splints, brace, hospital beds, oxygen equipment, commode, thermometer, sphygmomanometer, and stethoscope.
