The primary purpose of your job is to assure optimum elder care is provided for all elders of the Tabitha Residence. This includes coordinating, planning, organizing, implementing, supervising, and evaluating elder care provided by the staff assigned to care for your elder assignments and to assure it is performed in accordance with their job levels, abilities, and in compliance with the policies and procedures of Tabitha, National Greenhouse Project, state/federal regulations standards and as may be required by the Director of Nursing.
This position is also responsible for ensuring licensed nurse coverage to Tabitha Nursing and Rehabilitation Center while not in the Tabitha Residences. This could include, but is not limited to, assuming responsibility for direct patient care or providing coverage for Care Managers while off the floor.
This position is responsible for working to fulfill the goals and objectives of the Residences and Tabitha Nursing and Rehabilitation Center. Responsible for delivery of quality service, excellent customer service, promoting good public relations, budget compliance, and for upholding the mission and standard of the Greenhouse and the facility.
Ensures optimum elder care by providing, planning, organizing, supervising, directing, and evaluating elder care.
Coordinates overall continuity of elder care by assisting, directing, and monitoring nursing staff on unit, in accordance with the care plan, assuring that policies and procedures are followed and that state/federal regulations and standards are carried out. Cooperate fully during the survey process and takes immediate, appropriate action to correct deficiencies.
Facilitates care plan meetings ensuring elders/families and appropriate disciplines are included as appropriate.
Maintains a team approach with Shahbaz/staff through the use of praises and constructive criticism.
Supervises staff and recommends disciplinary action as necessary or voices concerns to Assistant Director of Nursing.
Develops and maintains positive customer service with inter-department personnel, as well as other departments within the organization to ensure that nursing activities can be adequately maintained to meet the needs of the elders.
Notes and follows through on elder's change of condition (improvement or deterioration) according to facility policy, including assuring contact with and updating family, physician, and appropriate other disciplines.
Provides directions and monitoring of medications for elders who are receiving medications by Care Partners/Medication Aides.
Coordinates daily schedule between Tabitha Nursing and Rehabilitation Center and Residences to ensure adequate coverage is being delivered.
Will attend meetings and provide information needed as a collaborative effort as a member of the Residences Clinical Support Team.
Demonstrates a working knowledge of Tabitha Residences & Tabitha Nursing and Rehabilitation Center policies and procedures.
Assures accuracy of elder medical records consistent with the comprehensive Care Plan and Nursing policies and procedures.
Responds appropriately to emergencies. Implements appropriate safety and disaster measures for the protection of clients, visitors and staff.
Evaluates, implements and monitors the facility infection control measures including, but not limited to, hand washing, Standard Precautions, disposal of infectious waste and the use and storage of cleaning chemicals.
Evaluates, implements and monitors the facility safety measures, including, but not limited to, use of gait belts and protective equipment, side rail application and the use of restraints, and prompt reporting and repair of malfunctioning equipment.
Ensures delivery of optimum elder care by being to work on time, as scheduled, and/or providing appropriate advance notification.
Attends mandatory and non-mandatory in-services for the Tabitha Residences & Tabitha Nursing and Rehabilitation Center, workshops and training sessions to maintain continuing education.
Performs other duties and special projects as assigned by your supervisor.

COMMUNICATION: Must be able to speak, read, write, and understand the English language including medical terminology related to the job. PHYSICAL REQUIREMENTS: Ability to gain access to the work area and then safely perform the duties of the job while utilizing standard and specialized equipment, including but not limited to the equipment described below. Physical stamina with the ability to perform duties while moving about the work area for the entire shift with few opportunities to sit down and with little or no break between tasks. Strength and agility sufficient to frequently and continuously lift, push, pull, reach, squat, kneel, bend, carry, twist, pinch, grasp, climb stairs while performing duties (such as bathing, toileting, transferring) and manipulating various supplies and equipment. Frequent periods of intense activity. Ability to safely perform unassisted transfers (from/to bed/chair/toilet/commode/bathtub/shower) and otherwise provide care to elders/clients who are weak, frail, obese, unable to assist due to limited mobility, quadriplegia, paraplegia, tremors, paralysis etc. Ability to remain calm and work effectively when others (such as elders/clients, coworkers or family members) experience emotional extremes, up to and including elders/clients who become combative. Vision and hearing within normal limits (with or without correction). Must have unrestricted lift/push/pull ability and pass a 50 - pound physical exam demonstrating strength and agility prior to employment and at Tabitha’s discretion.
Works indoors in well lighted and ventilated surrounds. Will require frequent trips between Tabitha Residences and Tabitha Nursing and Rehabilitation Center. Sits, stands, and walks intermittently during working hours. Is subject to frequent interruptions. Is involved with elders, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances. Is subject to hostile and emotionally upset elders, family members, personnel, and visitors. Communicates with the medical staff, nursing personnel, and other department supervisors. Works beyond normal working hours and on weekends and holidays when assigned. Is subject to injury from falls, burns from equipment, odors, sprains and strains due to moving and lifting residents, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants. Is subject to exposure to infectious waste, diseases, conditions, etc., including but not limited to TB and the AIDS and Hepatitis B viruses. May be subject to the handling of and exposure to hazardous chemicals. Maintains a liaison with the elders, their families, support departments, etc., to adequately plan for the residents' needs.
Computer, telephone, copy machine, electronic and regular scales, thermometers (electronic, regular, and tympanic), blood pressure apparatus, oxygen administration apparatus (tanks and concentrators), suction machine, infusion pumps (IV, Sub-Q, and tube feeding), hoyer/mechanical lift, EZ pivot, whirlpool bath and chair lift, ambulation equipment (walker, cane, crutches, etc.), wheelchair, gerichair, merry walker, specialized beds, Aqua K pad, heat lamp, humidifier, oximeter, glucometer, doppler, gait belts, bedpan, urinals, physical restraints and/or enablers, commodes, shower chairs and other equipment as deemed necessary in providing resident care.
Must possess a current, unencumbered, active license to practice as an RN/LPN in the State of Nebraska.
Current Healthcare Provider or BLS CPR/AED certification required.
EDUCATION/TRAINING AND WORK EXPERIENCE
Graduate of an accredited School of Nursing approved by the State of Nebraska.
One year experience as a staff nurse and/or team leader preferred.
