Responsibilities

                                                                 

JOB SUMMARY

This position is responsible for observing, implementing, and documenting nursing care provided to assigned clients. This position is responsible for delivery of quality service, promoting good public relations, and for upholding the mission and standards of the facility.

 

ESSENTIAL FUNCTIONs

  1. Provides skilled observation and/or care to clients requiring intermittent professional nursing services under the direction of the primary RN.
  2. Maintains clinical record documentation according to agency policy and procedures and state/federal regulations.
  3. Adheres to the client’s Home Health Care Bill of Rights.
  4. Coordinates the client's care and ensures that appropriate treatment is obtained.
  5. Provides reports to primary nurse and other staff involved in care.
  6. Provides on-call and weekend nursing visits on rotating basis in assigned.
  7. Adheres to has knowledge of HHC policies and procedures.
  8. Adapts to a changing work load and work environment.
  9. Follows all safety and infection control policies and practices established by Home Health Care and regulatory agencies.
  10. Organizes and prioritizes work to complete responsibilities in established time frames.
  11. Participates in organizational and departmental goals and performance improvement activities.
  12. Knowledgeable and adheres to disaster procedures during drills and actual emergencies, e.g., fire, tornado, bomb threat, and evacuation.
  13. Assures delivery of optimum service by following assigned schedules.
  14. Participates in personal and professional growth by attending inservice/educational workshops and scheduled department meetings.
  15. Assists in maintaining Tabitha's positive image in the community.
  16. Maintains a positive outlook toward employer and work responsibilities.
  17. Dresses and conducts self in an appropriate manner according to HHC Agency policy.
  18. Performs other responsibilities and special projects as assigned by the supervisor.

Qualifications

LICENSURE/CERTIFICATION

Current Nebraska Licensed Practical Nurse licensure.

Current Healthcare Provider or BLS CPR/AED certification required.

Valid Nebraska driver's license and state required minimum automobile insurance coverage required.

 

EDUCATION/TRAINING, AND WORK EXPERIENCE

Graduation from NLN accredited school of practical nursing required.

One year of acute or skilled care nursing experience within the last two years required.

 

KNOWLEDGE, SKILLS AND ABILITIES

Knowledge of and ability to apply practical/vocational nursing principles and practices.  Knowledge of teaching/learning principles, environmental hazards and good safety practices.

Ability to:

- exercise independent judgment

- perform CPR

- assemble records and present data in a concise, organized, systematic manner

- teach clients and families

 

WORKING CONDITIONS

Works in clients' places of residence including apartments, town houses, duplexes, and houses.  May care for clients with infectious disease so must observe Standard Precautions/Infection Control procedures.  Travel is required from residence to residence in all kinds of weather.  Must have a reliable means of getting to each client's residence on a timely basis.  May be exposed to pets and tobacco smoke in client’s residence

 

PHYSICAL REQUIREMENTS NEEDED TO PERFORM THIS JOB

Must have ability to lift up to 50 pounds, twist upper body and bend at waist while transferring, moving, turning and supporting clients.

 

EQUIPMENT OPERATION

Use of computer for clinical record documentation.  Use of a pager.  Use of home blood glucose monitoring equipment, standing and chair scales, vascular pump, pocket Doppler, mechanical lift (e.g. Hoyer), wheel chair, walker, cane, splints, brace, commode, thermometer, sphygmomanometer, and stethoscope.
