Responsibilities

JOB SUMMARY

This position is responsible for providing Physical Therapy services to clients with physical or cognitive disabilities to maximize functional independence. This position is responsible for delivery of quality service, promoting good public relations, and for upholding the mission and standards of the facility.This position is responsible for supporting Tabitha’s philosophy of L.I.V.E:  Love your job, Invite optimism, Vision success, Embrace the Mission.

 

ESSENTIAL FUNCTIONS

  1. Demonstrate commitment to Tabitha’s Crew Code of conduct.

  2. Evaluates clients utilizing appropriate diagnostic muscle, nerve, joint and functional ability tests.

  3. Performs Physical Therapy treatment procedures or delegates to a licensed Physical Therapist Assistant based on initial and on-going client evaluation.

  4. Develops and implements the client's plan of care in collaboration with the physician, other consulting health care professionals, family/caregiver according to agency procedures and state and federal regulations. 

  5. Documents the care provided, instructions given, client's progress toward goals, and changes in condition adhering to Tabitha's policies and procedures and state/federal regulations.

  6. Reports significant changes in the client's condition to the physician and nursing staff.

  7. Communicates and coordinates care and client's progress with other team members involved in the care.

  8. Designs, implements, and evaluates therapeutic exercise programs, and instructs the client's family/caregiver/Home Health Aide in carrying out the program.

  9. Consults with the attending physician and obtains a written telephone order for Physical Therapy plan of care and any changes in the client's plan of care as needed.

  10. Ensures that assigned client’s clinical records are complete, timely, and reflect coordination of care.

  11. Oversees and coordinates Physical Therapist Assistant and Rehabilitation Aide client care activities.

  12. Organizes and prioritizes work to complete responsibilities according to plan within established time frames to meet the client’s medical and social needs.

  13. Communicates effectively with clients, families, staff and support services to achieve client goals and meet service expectations.

  14. Provides client care in a service environment that demonstrates honesty, integrity, compassion and trust.

  15. Promote positive client/staff relations according to the mission statement of Tabitha Health Care Services.

  16. Follows all policies and procedures for Tabitha Health Care Services and Home Health Care.

  17. Assures delivery of optimum service by reporting to work on time as scheduled, and/or providing appropriate advance notification.

  18. Participates in personal and professional growth by attending mandatory inservices and scheduled department meetings and seminars.

  19. Follows all safety and infection control policies and practices of the facility and regulatory agencies at all times.

  20. Maintains work schedule which allows for coverage in all Rehab. programs as needed.

  21. Attends and actively participates in client care conferences as indicated whenever on duty.

  22. Maintains a positive outlook toward employer and work responsibilities.

  23. Dresses and conducts self in an appropriate manner according to established procedures.

  24. Plans, coordinates, supervises, and evaluates all student interns and volunteers.

  25. Performs other duties and special projects as assigned by supervisor.

     

    NONESSENTIAL FUNCTIONS

  1. Participates in designated department/corporate committees, (example: Safety Committee, Performance Improvement Committee).

  2. Teaches in-services relating to Rehabilitation upon request of supervisor and in coordination with Director of Education.

  3. Assists in orientation of nursing employees relating to Rehabilitation.

Qualifications

LICENSURE/CERTIFICATION

Current Nebraska Physical Therapy license.

Valid Nebraska driver's license and state required minimum automobile insurance coverage required. Current C.P.R. certification required.

 

EDUCATION/TRAINING AND WORK EXPERIENCE

Bachelor's Degree in Physical Therapy required.

One year of clinical experience preferred.

Geriatric experience preferred.

 

KNOWLEDGE, SKILLS, AND ABILITIES

  • Ability needed to understand the challenges experienced when working with the geriatric client.

  • Personal traits needed are patience, tact, persuasiveness, flexibility, consistency, resourcefulness and emotional stability to help clients and their families understand the treatments and cope with their disabilities. 

  • Self-motivation and strong interpersonal skills are needed also.

 

WORKING CONDITIONS

Works in clients' places of residence including apartments, town houses, duplexes, and houses.  May care for clients with infectious disease so much observe Standard Precautions/Infection Control procedures.  Travel is required from residence to residence in all kinds of weather.  Must have a reliable means of getting to each client's residence on a timely basis.  May be exposed to pets and tobacco smoke in client's residence.

 

PHYSICAL REQUIREMENTS NEEDED TO PERFORM THIS JOB

Requires manual dexterity and physical strength/endurance to stoop, kneel, crouch and stand for extended periods of time while performing client care. 

Requires frequent lifting up to 40 pounds to move, lift, ambulate, and transport clients and equipment.

 

EQUIPMENT OPERATION

Continuous use of:  Physical assistive devices for evaluation/treatment including walkers, canes, splints, braces, crutches, wheelchairs, goniometer, ultrasound, dynamanometer, and electrical stimulation devices. Frequent use of: Laptop, computer and telephone.
