southeast Nebraska.

Responsibilities

JOB SUMMARY

This position is responsible for quick response, evaluation and direct care of clients and residents of Tabitha managed housing units on a PRN basis; for providing continuity of care on a 24-hour basis; for coordinating services for referrals after hours; and for providing program information as needed.  This position is responsible to work a minimum of 20 shifts per month which includes every other weekend and assigned holidays. This position is responsible for delivery of quality service, promoting good public relations, and for upholding the mission and standards of the facility.

 

ESSENTIAL FUNCTIONS

  1. Provides after hours care to home health care clients and to residents of Tabitha managed housing units.
  2. Admits clients for services and completes appropriate admission documentation according to agency policy and procedures and state/federal regulations.
  3. Provides skilled observation and/or care to clients requiring intermittent professional nursing services.
  4. Adheres to the Clients’ Home Health Care Bill of Rights.
  5. Maintains clinical record documentation according to agency policy and procedures and state/federal regulations.
  6. Coordinates the client’s care and ensures that appropriate treatment is obtained.
  7. Demonstrates and promotes communication skills.
  8. Teaches and supervises home health aides, family members, and non-paid caregivers providing client care according to agency procedures and state/federal regulations.
  9. Provides holiday staffing as requested.
  10. Follows all safety and infection control policies and practices established by home health care and regulatory agencies.
  11. Knowledgeable and adheres to disaster procedures during drills and actual emergencies, e.g. fire, tornado, bomb threat, and evacuation.
  12. Adheres to and has knowledge of HHC policies and procedures.
  13. Participates in organization and departmental goals and performance improvement activities.
  14. Participates in personal and professional growth by attending inservices/educational workshops and scheduled department meetings.
  15. Organizes and prioritizes work to complete responsibilities in established time frames.
  16. Assures delivery of optimum service by following assigned schedules.
  17. Adapts to changing work load and work environment.
  18. Maintains a positive outlook toward employer and work responsibilities.
  19. Assists in maintaining Tabitha’s positive image in the community.
  20. Dresses and conducts self in an appropriate manner according to HHC Agency policy.
  21. Performs other responsibilities and special projects as assigned by the supervisor.

Qualifications

LICENSURE/CERTIFICATION

Current Nebraska Register Nurses licensure.

Valid Nebraska driver's license and state required minimum automobile insurance coverage.

Current Healthcare Provider or BLS CPR/AED certification required.

 

EDUCATION/TRAINING, AND WORK EXPERIENCE

Graduation from NLN accredited school of nursing required.

BSN preferred. Two years of acute care nursing experience within the last five years required. Experience or education in community health preferred.

 

KNOWLEDGE, SKILLS AND ABILITIES

Excellent physical assessment skills required.

Knowledge of and ability to apply community health nursing principles and practices.

Knowledge of teaching/learning principles, environmental hazards and good safety practices.

Ability to: exercise independent judgment, supervise and assign work to others, perform CPR, assemble records and present data in a concise, organized, systematic manner, and teach clients and families.

 

WORKING CONDITIONS          

Works in clients' places of residence including apartments, town houses, duplexes, and houses.  May care for clients with infectious disease so must observe Standard Precautions/Infection Control procedures.  Travel is required from residence to residence in all kinds of weather.  Must have a reliable means of getting to each client's residence on a timely basis. May be exposed to pets and tobacco smoke in client’s residence.

 

PHYSICAL REQUIREMENTS NEEDED TO PERFORM THIS JOB

Must have ability to lift up to 50 pounds, twist upper body and bend at waist while transferring, moving, turning, and supporting clients.

 

EQUIPMENT OPERATION

Use of computer for clinical record documentation.  Use of Cellular phone and pager.  Use of home blood glucose monitoring equipment, standing and chair scales, home ventilator, infusion pump, vascular pump, pocket Doppler, mechanical lift (e.g. Hoyer), wheel chair, walker, cane, splints, brace, commode, thermometer, sphygmomanometer, and stethoscope.
