Responsibilities

JOB SUMARY

The Advanced Practice Nurse Practitioner is responsible for the completion of face to face client visits for the purpose of determining continued eligibility criteria for hospice care in collaboration with the Hospice Medical Director; provides comprehensive client assessment, acts as a primary nurse, completing evaluation visits and on going hospice care as assigned; assists with community and facility educational presentations and public relations tasks, as assigned. This position is responsible for delivery of quality service, promoting good public relations, and for upholding the mission and standards of the facility. ESSENTIAL FUNCTIONS

  1. Demonstrates commitment to Tabitha’s Crew Code of Conduct
  2. Provides client assessment and face to face visit for the purpose of establishing hospice eligibility criteria and documentation of criteria to ensure regulatory compliance. 
  3. Ensures the timely and adequate delivery of Hospice services to clients, including responsible for care plan management and oversight.
  4. Provides for development of the initial plan of care, evaluates client for any immediate needs such as medications, durable medical equipment or supplies and arranges for delivery.
  5. Maintains clinical record documentation according to agency policy/procedures and regulations.
  6. Actively participates as an Interdisciplinary team member, including offering insight and suggestions for care for all clients served by the team.
  7. Contributes to continuity of care and effective communication between client/family, Hospice team members, Home Health Agencies, Inpatient Facilities, Ancillary Services, and Physicians.
  8. Accepts responsibility of departmental and professional growth and development by evaluating delivery of care and serving as a role model.
  9. Accepts responsibility for providing on call, holiday and weekend nursing visits to Hospice clients, on an as needed basis during usual circumstances.
  10. Supports and interprets Tabitha Hospice and departmental philosophy and objectives.
  11. Participates as a team member to ensure that the department work loads and goals are met.
  12. Demonstrates professionalism through appropriate dress and conduct.
  13. Maintains knowledge of and adheres to established organizational and departmental policies and procedures.
  14. Participates in organizational departmental goals and performance improvement activities.
  15. Adapts to changing work load and work environment.
  16. Organizes and prioritizes work to complete responsibilities according to plan and with established time lines.
  17. Maintains a positive outlook toward employer, work responsibilities and customers.
  18. Follows safety and infection control policies and practices of the organization.
  19. Assures delivery of optimum service by following assigned work schedule.
  20. Participates in personal and professional growth by attending mandatory in-services and scheduled department meetings.
  21. Performs other duties and special projects as assigned by the supervisor.

 

NONESSENTAL  FUNCTIONS

  1. Participates in agency and community health programs to promote growth and understanding of hospice and home health services upon supervisor’s request.
  2. May attend funerals or memorial services of clients.

Qualifications

LICENSURE/CERTIFICATION

Current Nebraska APRN licensure.

Valid Nebraska driver’s licensed and state required minimum automobile insurance coverage required.

Current Healthcare Provider or BLS CPR/AED Certification required.

 

EDUCATION/TRAINING, AND WORK EXPERIENCE

Graduation from NLN accredited school of nursing required.

APRN Nurse Practitioner required

Two years of acute care/long term care nursing experience within the last five years required.

Experience in community health and/or hospice preferred.

 

KNOWLEDGE, SKILLS AND ABILITIES

Excellent physical assessment skills required.  Knowledge of and ability to apply hospice nursing principles and practices.  Knowledge of teaching/learning principles, environmental hazards and good safety practices.  Ability to:

-          exercise independent nursing judgment

-          supervise and assign work to others

-          perform CPR

-          assemble, record, and present data in a concise, organized systematic manner

-          teach clients and families

 

WORKING CONDITIONS

Works in clients’ places of residence including apartments, town houses, duplexes, and private residences.  May care for clients with infectious disease so must observe universal precautions/Infection Control procedures.  Travel is required from residence to residence in all kinks of weather.  Must have a reliable means of getting to each client’s residence on a timely basis.  May be exposed to pets and tobacco smoke in client’s residences.

 

PHYSICAL REQUIREMENTS NEEDED TO PERFORM THIS JOB

Must have ability to lift up to 50 pounds, twist upper body and bend at waist while transferring, moving, turning, and supporting clients.  Travel required with the communities served.  Must have ability to be mobile within the assigned service area.

 

EQUIPMENT OPERATION

Use of cellular phone and/or pager, may have notebook computer  for documentation.  Use of home blood glucose monitoring equi9pment, standing scales, infusion pump, mechanical lift (e.g., Hoyer), wheel chair, walker, cane, splints, brace, hospital bed, commode, thermometer, sphygmomanometer, stethoscope, and oxygen equipment.
