Responsibilities

  1. Maintains the active clinical record for inpatient programs by printing, filing, thinning, or replacing chart forms as needed.
  2. Assists nursing with transcribing and performing data entry of signed physician orders to include assisting with admissions.
  3. Performs chart analysis to assure that necessary elements are present and complete.
  4. Assists with necessary paperwork or data entry to facilitate Senior/client admissions, transfers, and discharges.
  5. Schedules appointments for Seniors/clients, makes arrangements with family if appropriate, and schedules van transportation when needed.
  6. Oversees the operations of transporting residents. Schedules transportation to effectively cover any appointments.
  7. Coordinates to schedule new TEAMember orientation, compliance training and meetings.
  8. Analyzes staffing patterns and communicates staffing needs to the DON/Administrator.
  9. Monitors frequency of physician visits and notifies physicians of upcoming regulatory required visits.
  10. Creates, approves and maintains schedules and on call calendars in scheduling software for living community nursing departments.
  11. Uses problem solving techniques to suggest creative scheduling strategies as needed for living community nursing departments.
  12. Regularly assumes scheduled on-call duties which will include a week long (7 day) obligation on a rotating basis.
    • Frequency may vary depending on how many managers are sharing these duties and whether there are extenuating circumstances which result in the need for additional on-calls shifts with very little or no notice.
    • While on call, may receive phone calls at any hour about Senior/client change in condition, questions concerning admissions/discharges, Senior/client/family concerns, staffing needs, schedule changes, etc.
    • May be able to answer such questions over the phone or may need to return to Tabitha to handle situations that arise.

Qualifications

COMMUNICATION:  Must be able to speak, read, write, and understand the English language as well as terminology and jargon specific to the job. Ability to recognize and interpret non-verbal cues.
PHYSICAL REQUIREMENTS:  Ability to gain access to the work area and then safely perform the duties of the job while utilizing standard and specialized equipment. Vision and hearing within normal limits (with or without correction). Must have unrestricted lift/push/pull ability and pass a 50 - pound physical exam demonstrating strength and agility prior to employment and from time to time thereafter at Tabitha’s discretion.    
EQUIPMENT USAGE: Standard and specialized equipment including but not limited to gait belts, bedpans, urinals, wheelchairs, geri - chairs, commodes, shower chairs, blood pressure apparatus, thermometers, ambulation equipment, whirlpool, scales, oximeter, glucometer, Doppler, suction machine, infusion pumps (IV and tube feeding) oxygen concentrator, mechanical lift devices, Clinitron bed, Aqua-K pad, braces, splints, side rails, humidifier, fan, walkie talkie, tablet, telephone, computer/laptop, electronic devices, fax machine and photocopier.
AGE REQUIREMENT: At least age 18.
EDUCATION: Successful completion of a Licensed Practical Nurse course of study required.
EXPERIENCE: 6 months experience as an LPN preferred. Medical record experience in a long-term care facility, hospital, or medical clinical preferred.
CREDENTIALS: LPN License in good standing as recorded on the Nebraska State Registry or other Nurse Compact state required. CPR required.  All licenses, certifications, and permits must be kept up to date at the expense of the TEAMember. Valid Nebraska Driver’s license (unless otherwise allowed by law) and a good driving record required. Must possess auto insurance at or above Nebraska statutory requirements. Must meet Tabitha’s Vaccination Requirements.
