Responsibilities

POSITION SUMMARY:                                    

The Hospice Spiritual Care Coordinator is responsible for the provision of spiritual care services to Seniors/clients/residents (hereinafter, “Seniors/clients”) and families/caregivers of Tabitha Hospice, either directly or through coordination of care with other spiritual counselors. This TEAMember strives to uphold Tabitha’s LIVE2Care culture and actively embraces Tabitha’s Core Values.  As a result, the TEAMember supports Tabitha goals related to dignity, respect, independence, competence, autonomy, personal comfort, safety and security of Seniors/clients.  In addition, the TEAMember contributes to the success of Tabitha through ongoing collaboration with, TEAMembers, management, volunteers, and others in the organization.

 

ESSENTIAL FUNCTIONS:

  1. Assures spiritual assessment of Seniors/clients and families/caregivers in the hospice program and appropriate services and rendered in a timely manner.
  2. Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
  3. Provides direct spiritual care to Seniors/clients and families/caregivers.
  4. Serves as liaison and support to community chaplains and spiritual counselors.
  5. Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
  6. Documents direct services and ongoing communication with community chaplains and spiritual counselors.
  7. Attends Senior/client care conference and interdisciplinary group meetings as a member of the interdisciplinary group.
  8. Provides consultation, education, and support to the interdisciplinary group on spiritual care.
  9. Recruits community chaplains, spiritual counselors, and spiritual care volunteers adequate to meet Senior/client and family/caregiver needs by developing community contacts and offering education through congregations about hospice care.
  10. Provides for funeral or memorial services for Seniors/clients as requested.
  11. Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
  12. Assists as needed in evaluating and planning services to meet Hospice personnel support needs, i.e., yearly retreats, staff bereavement, team building, support groups and general care.
  13. Performs other duties and special projects as assigned.

Qualifications

COMMUNICATION:  Must be able to speak, read, write, and understand the English language as well as terminology and jargon specific to the job. Sufficient fluency to be able to understand and follow written and verbal instructions, provide instructions to others, read and understand policies, procedures or other similar documentation, interact successfully with Seniors/clients/caregivers, hold a conversation, conduct or participate in meetings, compose professional e-mails or other written communication, prepare legible, accurate and thorough documentation as required by the job and effectively interact with TEAMembers, customers, vendors, volunteers, managers and family members.  Ability to recognize and interpret non-verbal cues.  Ability to respond calmly and clearly to emergencies and emotional situations.


PHYSICAL REQUIREMENTS: Ability to gain access to the work area and then safely perform the duties of the job while utilizing standard and specialized equipment, including but not limited to the equipment described below.  Ability to move about the work area. Must have unrestricted lift/push/pull ability and pass a 25 - pound physical exam demonstrating strength and agility prior to employment and from time to time thereafter at Tabitha’s discretion.


AGE REQUIREMENT: At least age 18.


EDUCATION: Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree) preferred.


EXPERIENCE: Minimum of three years combined experience in pastoral care, spiritual services or program coordination preferred.  


CREDENTIALS: Affiliation with Evangelical Lutheran Church in America, Methodist, United Church of Christ, Episcopalian or Presbyterian preferred. Valid Nebraska Driver’s license (unless otherwise allowed by law) and a good driving record required. Must possess auto insurance at or above Nebraska statutory requirements. Must meet Tabitha’s vaccination requirements.
